What the program funds

Tennessee’s Rural Health Transformation Program puts federal money behind closing the gap in rural care. For behavioral health, SUD and I/DD providers, the practical barrier is rarely the will to modernize — it is the cost and disruption of moving off the system you are on. That is what this is meant to absorb.

Implementation and data migration

Role-based training for clinical and front-office staff, with support through the first weeks of live use.

TNWITS reporting without double entry

MindWise is currently the only EHR with a production TNWITS integration, so state reporting comes off the record your clinicians already documented in.

Discounted rural licensing

A reduced per-user rate for rural providers, which MindWise intends to maintain beyond the grant period rather than reset when funding ends.

The 89-county rule

Every Tennessee county is designated rural for this program except the six with locally governed metropolitan health departments — Davidson, Hamilton, Knox, Madison, Shelby, Sullivan. Because the line follows public health governance rather than population, several populous counties are inside it. Do not assume from how rural a county feels.

Common questions

Which counties count as rural?
89 of Tennessee's 95 counties. The exceptions are the six that run their own metropolitan health departments: Davidson, Hamilton, Knox, Madison, Shelby, Sullivan. The designation follows public health governance rather than population, which produces some results people do not expect — Rutherford, Williamson, Montgomery and Sumner are all included, while Sullivan is not.
Our head office is in Nashville but our sites are rural. Do we qualify?
Eligibility follows where services are delivered, not where the head office sits. Select every county you operate in on the form and the screening will pick up the qualifying ones.
Is Jackson eligible?
It depends which Jackson. The city of Jackson is in Madison County, which is outside the designated counties. Jackson County, in the Upper Cumberland, is inside them. This one catches people out regularly, so it is worth checking which you mean.
Does a positive result mean we have funding?
No. The form screens your county footprint and provider type against the program's designation — it is not a funding decision. TDH determines final eligibility, and any grant-funded onboarding depends on grant contracts being executed.
What if we are not eligible?
The grant program is one route to MindWise, not the only one. If your counties fall outside the designation we will still follow up, and the platform is available on its standard terms.
How long does onboarding take?
Most programs are live within four to six weeks including data migration. We map your actual timeline on the first call, once we know how many sites and which programs are involved.
